Allergic conjunctivitis (the medical name for allergy eyes) is one of the most under-treated allergic symptoms. People reach for OTC eye drops, get partial relief, and assume that is the ceiling. It is not. Allergy eyes almost always travel with allergic rhinitis, and treating the systemic allergy usually resolves the ocular component as a bonus.

What makes the eyes itch

Mast cells in the conjunctiva (the thin tissue covering the eyeball and inside the eyelid) release histamine when they encounter allergens. The chemistry is the same as in the nose, just more visible. The classic giveaway is itch. Bacterial conjunctivitis is sticky and crusty. Viral conjunctivitis is red and watery but not particularly itchy. Allergy is itchy enough that patients rub their eyes raw without thinking about it.

Central Texas patterns

Worst eye symptoms tend to come during oak pollen weeks (March through May), grass seasons (May through October), and ragweed (September and October). Mountain cedar in winter affects more nose than eyes for most patients, though some get significant ocular symptoms. Indoor allergens (dust mites, pet dander, mold) drive year round eye symptoms.

Treatment options

Topical antihistamine and mast cell stabilizer drops (olopatadine, ketotifen, alcaftadine) are first line. Cold compresses help with acute flares. For moderate to severe disease, prescription drops and treating the underlying allergic rhinitis usually do the heavy lifting. Immunotherapy is the only treatment that targets the underlying immune response and works well for ocular symptoms.

When to see a specialist

Severe symptoms, light sensitivity, vision changes, or eye pain need same-day evaluation by an ophthalmologist or allergist. Vernal keratoconjunctivitis and atopic keratoconjunctivitis are severe variants that can damage the cornea and need specialized care.
